What is Volvulus?

Volvulus is a serious medical condition where a part of the intestine twists around itself and the mesentery (the tissue that supports the intestine).

This twisting can block the flow of digestive contents, leading to severe complications. Volvulus can occur in various parts of the digestive tract, but it most commonly affects the colon (colonic volvulus) or the small intestine (intestinal volvulus).

This condition can cause intense abdominal pain, bloating, and, if not treated promptly, can lead to tissue death and other severe health issues.

Side Effects of Volvulus

The side effects and symptoms of volvulus can vary, but common ones include:

  • Abdominal Pain: Severe, crampy pain in the abdomen, often sudden in onset.
  • Bloating and Distension: Swelling of the abdomen due to trapped gas and fluid.
  • Nausea and Vomiting: Often accompanied by an inability to pass stool or gas.
  • Constipation or Obstipation: Difficulty or inability to have bowel movements.
  • Fever: If there is associated infection or inflammation.
  • Signs of Shock: In severe cases, symptoms such as rapid heartbeat, low blood pressure, and confusion may occur.

If not treated, volvulus can lead to serious complications like bowel ischemia (reduced blood flow to the intestine), perforation (a hole in the intestinal wall), and peritonitis (infection of the abdominal lining).

How is Volvulus Diagnosed?

Diagnosing volvulus involves several steps and medical tests:

  • Medical History and Physical Examination: Your healthcare provider will review your symptoms and perform an abdominal exam.
  • Imaging Studies:
  • Abdominal X-ray: Can reveal signs of intestinal obstruction or distension.
  • CT Scan: Provides detailed images of the abdomen to confirm the presence of volvulus and assess its severity.
  • Ultrasound: Sometimes used, particularly in children or pregnant women, to identify twisting and fluid buildup.
  • Endoscopy: In some cases, a flexible tube with a camera (endoscope) may be used to visualize the inside of the digestive tract.

Potential Treatment of Volvulus

Treatment for volvulus depends on the severity and the location of the twist. It generally includes:

  • Emergency Surgery: Most cases require surgical intervention to untwist the affected segment of the intestine and potentially remove any damaged tissue. This procedure is often performed as an emergency due to the risk of complications.
  • Endoscopic Decompression: In some instances, especially if diagnosed early, a gastroenterologist may use an endoscope to relieve the obstruction without surgery.
  • Supportive Care: Treatment may include intravenous fluids, pain management, and antibiotics if there is an infection.
  • Post-Surgical Care: After surgery, you may need to follow a specific diet and gradually reintroduce food as recommended by your healthcare provider.
